Position Summary:
The Wound/Ostomy Care Consultant is responsible for the coordination of patient care activities for a specific patient population by guiding them through the health care system, collaborating with physicians, providing the patient with support, education, indicated referrals, and other assistance during the patient care experience. Responsible for establishing and managing an ongoing relationship with the interdisciplinary team to facilitate a timely, integrated, and coordinated approach to care. Monitors and reports on quality indicators and performance improvement initiatives. Assumes a leadership role in the areas of patient and family education, and staff education and development. The position models the Rush Copley Medical Center mission, vision and values, the Care Model, and acts in accordance with Rush Copley Medical Center and nursing policies and procedure. Position Requirements: License: Must be licensed and currently registered to practice as a Registered Nurse or Advanced Practice Registered Nurse in the State of IL. Education: BSN required, MSN preferred. American Heart Association ACLS and CPR required Specialty certification required within 2 years of hire. Minimum 3 years of experience in nursing, wound care experience strongly desired. Excellent interpersonal, communication, problem-solving, autonomy, and decision-making skills. High level of analytical ability and the skill to work in complex clinical or hospital systems required. Experience in clinical program development and hospital operations preferred. Experience with EPIC preferred.
