**Position Summary:** The Wound/Ostomy Care Consultant is responsible for the coordination of patient care activities for a specific patient population by guiding them through the health care system, collaborating with physicians, providing the patient with support, education, indicated referrals, and other assistance during the patient care experience. + Responsible for establishing and managing an ongoing relationship with the interdisciplinary team to facilitate a timely, integrated, and coordinated approach to care. + Monitors and reports on quality indicators and performance improvement initiatives. + Assumes a leadership role in the areas of patient and family education, and staff education and development. + The position models the Rush Copley Medical Center mission, vision and values, the Care Model, and acts in accordance with Rush Copley Medical Center and nursing policies and procedure. **Position Requirements:** + License: Must be licensed and currently registered to practice as a Registered Nurse or Advanced Practice Registered Nurse in the State of IL. + Education: BSN required, MSN preferred. + American Heart Association ACLS and CPR required + Specialty certification required within 2 years of hire. + Minimum 3 years of experience in nursing, wound care experience strongly desired. + Excellent interpersonal, communication, problem-solving, autonomy, and decision-making skills. + High level of analytical ability and the skill to work in complex clinical or hospital systems required. + Experience in clinical program development and hospital operations preferred. + Experience with EPIC preferred.
