Wound Care Case Manager - Quality & Clinical Outcomes

The Wound Care Registered Nurse is a key member of the Quality Department, serving as a clinical expert in the prevention, assessment, and management of wounds—including pressure injuries—across the hospital setting. This position functions independently of frontline staffing assignments and focuses on hospital-wide performance improvement through proactive rounding, consultation, staff education, and data-informed practice.
This role leads Quality initiatives to reduce hospital-acquired pressure injuries (HAPIs), ensures regulatory compliance, and partners with interdisciplinary teams to elevate standards of care. The Wound Care RN supports frontline staff through coaching, mentorship, and education, but does not carry a routine patient assignment.
EDUCATIONAL REQUIREMENTS:
+ Graduate of an accredited School of Nursing
+ Current licensure as a Registered Professional Nurse in New York State
+ Bachelor of Science in Nursing (BSN) required; Master’s degree preferred
+ Board certification in Wound Care (e.g., WOCN, WCC) preferred
+ Current Basic Life Support (BLS) certification
+ Minimum 3 years of acute care nursing experience; wound care experience strongly preferred
PRIMARY RESPONSIBILITIES OF THE POSITION:
Performance Improvement & Quality Oversight
+ Collaborates with the Quality Department to lead pressure injury prevention strategies
+ Analyzes HAPI data, identifies trends, and partners with unit leadership to implement improvements
+ Leads or supports root cause analyses, prevalence studies, and audits related to wound care
+ Ensures regulatory readiness and assists with policy updates related to skin integrity and patient safety
Clinical Expertise & Education
+ Serves as a consultative resource on evidence-based wound care practices and prevention strategies
+ Educates and mentors staff on proper wound staging, dressing selection, and prevention protocols
+ Supports onboarding and competency validation efforts across inpatient and procedural areas
+ Develops and delivers targeted education sessions and unit-level coaching
Collaborative Rounding & Leadership
+ Conducts daily skin rounds house-wide to proactively identify risk and initiate intervention
+ Provides leadership and visibility to nursing teams as a subject-matter expert
+ Acts as liaison between clinical departments and Quality/Compliance on wound-related initiatives
Professionalism & Communication
+ Maintains documentation of assessments, consultations, and outcomes in accordance with policy
+ Communicates with nursing leadership and clinical teams to support data transparency and improvement planning
+ Represents the Quality Department on internal committees, workgroups, and quality forums

Salary Range: $ 36.07-54.10/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
