You Will Be Responsible For:
Clinical Practice: Assesses physical, psychological and educational needs of patients according to guidelines, standards of care and individual needs.
Assesses patient care needs and/or changes in condition throughout treatments and procedures and communicates findings to the provider.
Planning: Coordinates plan of care for patients, including, but not limited to, appointments, transportation, education, and follow up treatments to ensure timely delivery of diagnostic and treatment services.
Coordinates referrals to medical and radiation oncology, physical therapy, and other services as determined by providers.
Provide and coordinate education and information to patient and family to ensure a seamless, continuous, and comprehensive continuum of care.

40 hours, days
Requirements:
Current Connecticut RN Licensure.
Bachelor of Science in Nursing (BSN).
Minimum 2-4 years’ previous experience in a healthcare related field
Must have certification for CN-BN within 12 months of accepting this position, as required by NAPBC
