Vice President of Acute Case Management

GENERAL SUMMARY:
Under the direction of the Vice President, System Command Center the Vice President Acute Case Management is accountable for the strategic planning, development, implementation, and oversight of clinical initiatives and programs across Henry Ford Health System’s inpatient case management departments. In this capacity, the Vice President collaborates with senior leaders in HFH Markets, Hospitals, Physician groups, Health Plans, and Service Lines, and partners with the Vice President, Population Health Management, Clinical Integration, and Ambulatory Care Management to implement strategies and systems that ensure the coordination of care management services across the enterprise, resulting in consistent and seamless experiences for HFH patients and efficient and integrated processes and outcomes for providers and other stakeholders across the health system.
+ Accountable for the strategic development of Population Health Management initiatives that increase value to patients and reduce the total cost of care by standardizing and innovating new inpatient case management and post-acute care services within and across HFH hospitals and owned/contracted post-acute facilities and functions.
+ Leads a team of case management directors/managers and project managers responsible for case management and transition of care functions and improvements within, between, and among HFH hospitals and post-acute facilities or functions.
+ Collaborates with the Vice President of Populations Health Management, Clinical Integration and Ambulatory Care Management to develop and lead the systems integrated case management enterprise.
+ Leads process standardization initiatives designed to improve clinical quality, efficiency and financial performance of the organization through and integrated case management function. Responsible for establishing and reporting progress on relevant outcome and process measures tied to HFH strategic goals in quality/safety, experience and cost.

+ Seeks and maintains expertise in strategies and methodologies that exhibit improvement in the quality and effectiveness of care management service delivery across the continuum of care. Represent HFH at local and national conferences and collaborates on Population Health and integrated case management innovations.
+ Collaborates with local leaders to ensure inpatient and ambulatory care management initiatives and interventions meet State and Federal regulations and Direct to Employer and Health Plan contractual requirements.
+ Accountable for organizing and leading key stakeholders including hospital, post- acute and physician leaders to coordinate efforts and identify possible solutions and owners of initiatives or programs to ensure comprehensive patient assessments, improve discharge planning and transitions of care, reduce readmissions, and decrease the total cost of care.
+ Develops structures and processes to facilitate participation by physicians, pharmacists, nurses, case managers and other health professionals in multidisciplinary care planning, care delivery, utilization review and discharge planning.
+ Strengthens relationships with post-acute providers to ensure effective and efficient transitions of care between and among acute care, ambulatory/specialty providers, and post-acute providers.
+ Collaborates with key stakeholders to define and implement an organizational strategic plan to improve quality and efficiency metrics of post-acute providers.
+ Provides executive leadership to teams responsible for conducting systematic review of the organization’s functional capacity to implement new interventions for specific segments and cohorts of patient populations.
+ Leads and mentors others in data driven decision making.
+ Provides executive leadership to teams responsible for developing and maintaining communication systems supporting operational, resource and financial decision making.
+ Accountable for providing executive leadership for large population health management redesign projects to maximize effective resource stewardship.
+ Interacts regularly with executive management leaders and governing bodies across the enterprise to create strategies and report on initiatives and performance results related to Integrated Case Management.
EXPERIENCE REQUIRED:
+ Ten (10) years of increasing management responsibility in the health care industry required.
+ Experience developing and leading clinical interventions, departments and initiatives required.
+ Experience in developing partnerships and implementing quality improvement initiatives with post-acute care providers preferred.
EDUCATION REQUIRED:
+ Master’s degree in a nursing, social work, business, or a related healthcare field required.
CERTIFICATIONS/LICENSURES REQUIRED:
+ Current Nursing or Social Work licensure in the State of Michigan required.
