Sinai Hospital, a regional medical campus for George Washington School of Medicine and Health Sciences (GWSMHS), serves the people of Greater Baltimore as well as national and international patients. As a nonprofit teaching hospital, Sinai provides training for more than 140 residents and 400 medical students yearly.

Position Summary:

We are seeking a compassionate, highly skilled Nurse Practitioner (NP) or Physician Assistant (PA) to lead our post-discharge care services at Sinai Hospital of Baltimore. This role is responsible for providing follow-up care to patients recently discharged from the hospital, ensuring a smooth transition to outpatient management, preventing readmissions, and optimizing recovery outcomes. The NP/PA will serve as a key member of our multidisciplinary team, working closely with physicians, case managers, social workers, and care coordinators.

 

Full-time, hospital-employed, outpatient opportunity with benefits Schedule: 40 Hours a week, at Sinai Hospital Hours: Monday - Friday, 8:00 am to 4:30 pm or 8:30 am to 5:00 pm

 

Key Responsibilities:

Conduct comprehensive post-discharge assessments for patients recently released from inpatient care/ ED or Observation. Review, access, & develop individualized care plans in collaboration with the discharge attending, primary care provider, and/or specialists. Evaluate treatment efficacy and modify plans as needed. Evaluate & treat patients for potential signs of clinical deterioration or potential complications. Educate patients and families on medication regimens, symptom management, and follow-up care instructions. Work with support staff & social workers to supervise the coordination of appropriate follow-up appointments, labs, imaging, and referrals. Communicate with hospital teams, skilled nursing facilities, and home health agencies as necessary. Document all encounters and care plans in the electronic health record (EHR) in a timely and accurate manner. Identify & recommend quality improvement initiatives related to reducing hospital readmissions and improving transitional care outcomes. Responsible for developing practice workflow by working collaboratively with the practice manager, social worker, and division leaders to support clinic operations, related to post-discharge care.
  Recommend process and procedure enhancements, and operational improvements related to methods for patient flow, patient scheduling, referral management, and follow-up care plans.

Qualifications:

Education: Master’s in nursing from an accredited program. Licensure: Current and unrestricted license in Maryland; national certification as a PA or NP (FNP, AGNP, or ACNP preferred). Experience: Minimum 3 years of PA/ NP experience preferred, ideally in hospital medicine, primary care, or transitional care setting (TCM). BLS/ACLS certification Skills: Strong clinical assessment and diagnostic skills Excellent communication and interpersonal abilities Comfortable working independently in a fast-paced environment Proficient with EHR systems and care coordination processes
