**Job Description:** This position functions as a vital member of the healthcare team who contributes to the success of the practice by demonstrating customer/patient focus, effective communication, professionalism, and teamwork. Utilizes the nursing process to assess, plan, implement, and evaluate comprehensive care provided to selected patient populations and their families. This position is responsible for timely and effective management of patient calls requiring medical triage and coordinating care during the transition of care following an inpatient medical stay. Provides guidance, education and facilitates resources for the management of the at-risk population. This is a virtual position, serving a region of the health system. Education must be obtained from an accredited institution.
