Job Description Hours: Monday-Friday 8am-4:30pm (no weekends, no on-call, no holidays) A large hospital/healthcare system is seeking a highly skilled and adaptable Triage Registered Nurse (RN) to join their Outpatient Comprehensive Care/Infectious Diseases team. This role is part of a collaborative care model involving physicians, advanced practice providers (APPs), and medical assistants (MAs). This is an outpatient/clinic office-based setting, with most patient interactions occurring via phone, and in-person visits. This position will be approximately 50% phone triage, 50% hands on patient care. This person will be responsible for the following: - Clinic medication management - Injectables and vaccine coordination - Communicate with inpatient consult services and outpatient providers to ensure seamless continuity of care. - Manage a high volume of patient calls and Epic inbasket communications setting up appointments Deliver HIV prevention and care services, including counseling and connection to community resources & participate in Division of HIV, STD, and TB Services (DHSTS) clinical navigator trainings (training provided).
