SUMMARY (Basic Purpose of the Job)Provides support and assistance to LIFE participants and potential participants related to transportation activities. Maintains communication with driver during all transport activities.ESSENTIAL FUNCTIONSDevelops and maintains positive relationships with co-workers, patients, and their significant others, medical staff, and visitors. Creates a safe and therapeutic environment that supports the special needs of the populations served.Pursues opportunities for self-development, job enrichment, or advancing knowledge and skills. Assist with the development and complies with all organizational and departmental policies, procedures, standards and patient safety principles and implements changes utilizing appropriate channels.Assists participants and potential participants during transportation. Assists participant with necessary preparation prior to transportation. Assists participant or potential participant onto and off of van in accordance with Capital Health policies and safety procedures.Assists the driver to secure participant personal items and DME. Remains on van while transporting participants. Acts as primary communicator with center, participant, and support serv ices when vehicle is in transport. Accompanies participants during outside appointments.Observes, reports, and records changes in the participant’s condition and environment to appropriate staff as needed. Stays alert for signs and symptoms of pain or discomfort. Asks questions if participant can make their needs known, reports pain or discomfort to transportation coordinator.Takes measures to provide safe care to prevent injury to self and participants during transportation. Uses proper body mechanics. Adheres to accepted infection control practices. Maintains a clean, safe work environment.Shares information on participant abilities, progress and preferences, and communicates information on a regular basis. Participates in assessment and care planning meetings with the Interdisciplinary Team or in other work groups, family meetings, or planning sessions as requested.Establishes a positive rapport with patients, patients' family members, and other members of the LIFE care team and its network providers.Remains an active and committed member of Capital Health LIFE staff and participates in program activities as assigned. Participates in interdisciplinary team meetings, family meetings, staff meetings, in-service and training programs, daily briefing meetings, and Quality Improvement activities.Performs other duties as required.MINIMUM REQUIREMENTSEducation: High school diploma or GED.Experience: One year of experience in a healthcare or home health setting.
