Join our multidisciplinary transplant team as a Transplant Social Worker, where you’ll play a vital role in supporting patients and families throughout the transplant journey. This position involves conducting psychosocial evaluations and diagnoses for transplant recipients, both pre- and post-transplant. You’ll develop treatment goals, provide solution-focused counseling, and empower patients to overcome barriers to care. Acting as a patient advocate, you’ll coordinate referrals to community resources and ensure continuity of care.
Key ResponsibilitiesConduct psychosocial evaluations and diagnoses for transplant patients and their familiesDevelop individualized treatment goals and care plansProvide brief, solution-focused counseling as neededAdvocate for patients and facilitate access to community resourcesCollaborate with the transplant multidisciplinary team to support patient outcomesEnsure psychosocial needs are addressed throughout the transplant processMaintain accurate and timely documentationParticipate in on-call coverage as neededMaintain a safe and ethical working environment in accordance with NASW standardsQualificationsEducation
Required: Master’s Degree in Social Work or related field from an accredited institutionLicensure
Required: Limited Licensed Master’s Social Worker (LLMSW) actively pursuing LMSW licensure in MichiganPreferred: Licensed Master’s Social Worker (LMSW) with clinical designation in MichiganExperience
