Transplant Clinical Social Worker
**Employment Type:** Part time **Shift:** Day Shift **Description:** **Job Summary** Join our multidisciplinary transplant team as a **Transplant Social Worker** , where you’ll play a vital role in supporting patients and families throughout the transplant journey. This position involves conducting psychosocial evaluations and diagnoses for transplant recipients, both pre- and post-transplant. You’ll develop treatment goals, provide solution-focused counseling, and empower patients to overcome barriers to care.
