Job Description Job Summary
The Transfer Coordinator Emergency Medical Tech (EMT) functions as a facilitator for patient transport in markets where CHS facilities are supported by the Access Transfer Center. Using a multi-faceted approach to schedule and arrange patient movement through the healthcare continuum, the Transport Coordinator (TC) may direct various activities including but not limited to allocation of transport resources and associated transport logistics, for dispatch 911, Interfacility, post-acute, and discharge transport.
Essential FunctionsPerforms Access Center functions in a timely, customer centered and professional manner. Coordinates transport of Emergent, Urgent and Elective patients from all potential referral sources including but not limited to hospitals, clinics, nursing homes, and physician offices. Facilitates patient throughput to a final destination facility with an ongoing focus on quality, efficiency, providing follow up to all participants of the transfer. Operate efficiently in all Access Center software applications; i.e. Transfer Software, i.e. Transport Online Ordering (OLOS) Portal, CAD Systems, Cerner EHR, About TC Documentation, etc., documenting patient status updates in the Electronic Health Record, Access Center database and by phone. Critical thinking skills will be utilized to ensure every patient transfer promotes the well -being and safety of the patient. Works well with others and promotes teamwork by demonstration of good team member behaviors. Identifies and assists team members with heavier workloads. Serves as an example of appropriate staff behavior when on or off calls. Assesses and documents the nature, location of requests, and level of priority and then dispatches the appropriate transportation resource. Receives and processes information on a continual basis with numerous helicopter and ground ambulances already on transports. Notify the appropriate helicopter or ground dispatch center of a request and relay all pertinent transport information. Able to simultaneously listen and accurately document transport activities in the Access Center and applicable dispatch databases. Performs a full range of telephone, computer and / or radio operational duties. Provides input into the Access Center processes and protocols to improve patient movement across the system. Recommends process change if needed to create a more efficient customer centered process. Demonstrates knowledge of local, state and federal regulatory requirements, CHS Ethics and Compliance policies: and ensures compliance standards are met or exceeded at all times. Adheres to established policy and procedure and standards of care. Identifies and escalates issues through the established Chain of Command in a timely manner. Will be overseen on a consistent basis by Transfer Coordinator. Provides high-level customer service with all interactions. Ability to organize and prioritize multiple complex tasks while working effectively with other employees and external parties. Demonstrate initiative and problem solving skills while working with multiple and various departments within each hospital to foster relationships with referring facilities, physicians, hospital staff and EMS providers. This is a key member of the CHS Access Center Team and is expected to assist Access Center staff as requested or when a need has been identified. Monitors throughput, documenting status updates and communicating updates to all customers as defined by protocol. Coordinate and allocate the response of EMS resources utilizing computer and telephone technology to effectively reduce patient care delays associated with transfer processes by determining Emergent / Non Emergent resource allocation; and notifying the appropriate ground or air dispatch center for response to needed services.Performs other duties as assigned. Complies with all policies and standards.QualificationsH.S. Diploma or GED required 1-3 years of related clinical experience in an acute care setting required 0-2 years Some experience in Pre Hospital environment / Emergency Department, Critical Care or Acute Hospital setting preferred Knowledge, Skills and AbilitiesDetail Oriented Ability to multi task Ability to function in a fast paced, at times high stress work environment Computer literate – understands database navigation and online clinical documentation Working knowledge of protocol based EMS dispatch Ability to attentively listen and simultaneously Type 38 wpmLicenses and CertificationsCertified Medical Assistant (CMA)-AAMA required EMT - Emergency Medical Tech Certification required or PARAMEDIC - Paramedic – Licensed required FP-C - Flight Paramedic - Certified required EMT - Emergency Medical Tech required FP-C - Flight Paramedic - Certified in residing state required
