**Employment Type:** Full time **Shift:** **Description:** **$10,000 bonus** **Day One Benefits** **NO ON Call!** **Territory:** **Team 2 encompasses Berwyn, Broadview, Riverside, but also Chicago.** **_Position Purpose:_** The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation. **_What You Will Do:_** + Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation + Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy + Provides statistical/budgetary data to appropriate Palliative care program + Works collaboratively with all departments to ensure execution of quality delivery of services **_Minimum Qualifications:_** + Graduate of an approved nursing education program. + Current Registered Nurse licensure in the state of practice. + CPR certification + Hospice/Palliative Care certification preferred. + Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills, + Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills + Must be able to work independently and on a team + Ability to perform diversified duties with time limitations + Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care **_Position Highlights and Benefits:_** + _Medical, dental and vision insurance - day one benefits_ + _Short and long-term disability_ + _403b with matching contribution_ + _Generous paid time off PLUS 7 paid holidays_ + _Comprehensive orientation_ + _Tuition reimbursement up to $5,250 a year_ + _Pay Range $36.71-$58.71_ **Ministry/Facility Information:** Loyola Medicine Home Care and Hospice is a member of Trinity Health At Home, a national home care, palliative care and hospice organization serving communities in twelve states.
