Position Responsibilities:

Balances leadership responsibilities and caseload appropriately Serves as a resource for the care management specialists, placement specialist, and administrative coordinator, and covers when necessaryOversees the performance of the team and provides feedback to department leadership as appropriateConducts orientation of new staff; completes orientation check list in accordance with policy/procedureMonitors compliance with the processes for giving IMMs, MOONs, and any other regulatory documents ensure timeliness and appropriatenessMonitors and assists in the Discharge Appeal process and in the distribution of the Medicare Denial notificationsPromotes a service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff, and visitors.Addresses service problems with outside companies, including home health agencies, durable medical equipment companies, and other services Communicates with patients and families as necessary when issues about insurance coverage or post-acute care needs ariseDocuments and communicates information in accordance with department policy

Demonstrates sufficient computer skills to perform job junctions, checks voicemail and email as instructed

 

Qualifications:

High School Diploma or GED2 years health care experience preferred. Knowledge of care management functions.
