We at Fairview Health are looking for a Team Coordinator to be based out of the Fairview Clinic in Elk River, MN. This role is approved for 56 hours every two weeks As a member of the care team the team coordinator performs quality patient and family-centered care principles through performing a variety of high level functions within a care team to support the needs of the healthcare team and patients/families. The team coordinator manages workflow by coordinating and maintaining provider schedules, uses a variety of communication tools and systems to handle medical information requests and updates, communicates normal test results either through the direction of the RN or provider or by following established protocols, and works collaboratively with the assigned unit while handling multiple tasks, interruptions and changes. As a member of the care team, the team coordinator will maintain complete, timely and accurate documentation for each patient and continually look for opportunities to improve processes and workflow.
