As a reputable provider of exceptional care and services for adults and seniors, we are seeking a dedicated professional to join our subacute admissions care team, ensure outstanding patient experiences, and foster a vibrant and engaging environment.
Aurora on France TCU is a fast-paced, high-quality care facility conveniently located in Edina, MN, and connected to M Health Fairview Southdale Hospital. Our dedicated and talented team works collaboratively to ensure patients meet their goals of care and return to their homes in the community. As the TCU Case Manager, you will have the opportunity to be a part of a vibrant team, empower patients, and create lasting impacts while you work to achieve the goals of initiating positive transitions into the facility, advocating for the needs of our patients, and facilitating a smooth and safe discharge back into the community.
This TCU Case Manager position will be tasked to provide efficient and customer friendly services for our TCU patients.
Schedule includes
80 hours every two weeks
Day shift, no weekends
Responsibilities:
• Positively support the culture of the household (TCU unit) by leading with a solution focused mindset
• Manage a caseload of 21 TCU patients
• Facilitate a smooth admission into the facility for new patients
• Intakes for up to 8 new admissions a day
• Meet with new patients the day they admit to the facility to begin discharge planning
• Facilitate positive and safe discharges
• Order discharge equipment such as wheelchairs, walker, hospital beds, and adaptive equipment
• Set up services for a safe discharge such as home care, wound care, oxygen, IV infusions, and others as appropriate for the patient
• Identify, investigate, and resolve complaints and grievances on the unit.
• Serve as the patient’s advocate and oversee patient quality of life in the facility.
• Create an exceptional experience that prioritizes safety and service to improve the experience, health, and wellbeing of everyone.
