A Little More About What You Will be Doing
The Supportive Care Medicine (SCM) Triage Nurse provides initial triage assessment of inpatient referrals to the Supportive Care Medicine consultative service and helps to assess the urgency of new consults. This position will work with other members of the interdisciplinary team to help manage the department’s overall consultation workflow and be a primary resource/contact for referring clinicians and the patient’s clinical team. They will work with the Supportive Care Medicine team and the patient’s primary team to ensure that patients experience seamless transitions from inpatient to outpatient environments, assist with initial as well as follow-up visits for patients who are on-service, and function as an integral part of the interdisciplinary team. The Supportive Care triage nurse must possess strong communication skills, clinical acumen, critical thinking, thoughtful decision-making and organizing abilities, as well as interpersonal skills to facilitate teamwork and to support the SCM team and clinical staff.

Education:
Graduation from an accredited school of nursing requiredBachelor of Science in Nursing degree preferredLicense/Certifications:
Valid CA RN license requiredCertified Case Management (CCM) RN preferredExperience:
3 years of acute nursing experience requiredExperience in palliative care, oncology or critical care and/or project management preferred.Req ID : 8851
