Job Summary:

The Supervisor of Care Coordination is a working supervisor who also provides administrative support to maintain routine operations with the underlying objective of enhancing quality and cost-effective care. They will implement care coordination activities that align with the strategic goals of ICM to promote growth and development of staff to meet those goals set for the department and organization. They will collaborate with case management leaders to support the onboarding, education, and growth of team through communication, use of technology, and best practice, and maintenance of department policies and procedures in accordance with ICM standards and will serve as a resource to the Case Management Team.

Position Responsibilities:

Care Coordination- facilitates the role of Care Coordination and supports transition of patient care through the continuum. Ensures appropriate departmental operations for care coordination through assessment, documentation, multidisciplinary rounds, and patient advocacy are met.  Works collaboratively with leadership and team to assist in advocacy and implementation of practice models of care. 

Supports daily activities by ensuring payroll, staffing, scheduling, and assignments meet the standards set within the department.

Provides quality review of documentation to assure appropriate interventions, methods, and communication. Educates and facilitates a culture of continuous quality improvement. Medical record review for quality and process improvement. Collaborates with case management leadership to facilitate development of department targets and monitors process to meet goals. Identifies opportunities for improvement and communicates appropriately.

Supports ICM and global G&O through staged escalation and, recognition of barriers and issues.

Supports departmental and organizational improvement activities including Shared Governance, Operational Excellence, and/or additional committees.

Required Experience: Required: 

Minimum 5 years of care coordination experience.

Knowledge of applicable state, federal and third-party regulations and standards.

Excellent verbal and written communication skills, critical thinking, problem solving, conflict resolution, and de-escalation skills

Required Education: 

Graduate of an accredited School of Nursing with BSN or Graduate of an approved School of Social Work with a Master’s Degree.  
Training / Certification / Licensure: 

Licensure and/or certification from the State of New Jersey as an MSW or Registered Nurse. 

Case Management Certification Preferred

Annual Salary: $86,960 - $138,844
The actual salary/rate will vary based on applicant’s experience as well as internal equity and alignment with market data.
