Position Summary: Supervisor / Nurse Home Visitor (NHV) is a professional position. The Supervisor/NHV’s role is to make sure Nurse‐Family Partnership (NFP) nurse home visitors are following the NFP program with fidelity at the same time meeting goals of the implementing agency (Guthrie). The position includes 90% of the time as Supervisor responsible for using the NFP principles of supervision to all the nurse home visitors. The other 10% of their time will be as a Nurse Home Visitor carrying a small caseload not to exceed 2 ‐ 4 clients.
Education, License & Cert: BSN required. Valid PA nursing licenses required. Valid and current driver’s license
Experience: Candidate must possess five or more years of recent experience in maternal and/or child health, public health, or mental/behavioral nursing with children and families. Home visiting experience preferred. Also, must have a minimum of two years supervisory experience preferred.
Essential Functions: 1. Represents and adheres to the Nurse‐Family Partnership model in actions and verbally to both internal and external customers and colleagues. 2. Performs home visits with first‐time mothers assessing physical, emotional, social and environmental needs of these women and their families as it relates to the NFP domains. 3. Assists families to establish realistic goals for promoting their health and life course development by identifying and building on strengths. Evaluates women and family’s progress toward targeted goals, revises plan of care as appropriate. 4. Completes all required documentation in a complete, timely, and concise manner. 5. Demonstrates the ability to utilize good independent decision‐making skills concerning the off‐site implementation of program objectives and goals. 6. Facilitate one‐on‐one weekly reflective supervision with nurse home visitors and weekly team meetings, maintaining all documentation. 7. Actively participate in all conference calls with NSO and the funding source as required. 8. Complete Joint Visit Observations (MAPS) with each Nurse Home visitor on a quarterly basis, including reflective practice and staff development to provide primary support and appropriate oversight to the nurse home visitors and to build model strengths ‐based client centered culture. 9. Maintain and interpret quarterly reported created by the NFP data system. 10. Assess that nurses have most up to date information needed to support their clients effectively. (includes knowledge of local resources, as well as professional development in mental health, drug and alcohol, motivational interviewing, breastfeeding etc.)
Other Duties: 1. Travel for this position is required. 2. Participation in community and employee engagement activities is required. 3. Assumes responsibility for ongoing learning in relations to program implementation. 4. Actively promotes program within the community. 5.
