Supervising Case Manager Community Home Base & Intensive Home Base Programs

Primary Purpose of Job:

The Supervising Case Manager is responsible for assisting the Program Director in managing the day-to-day operations of the Home Base Program. The Supervising Case Manager will provide crisis intervention and case management services for client families in the Home Base Program.

 

 

Principal Duties & Responsibilities:

Assist families in resolving the crises and problems that led to PINS and/or juvenile delinquency through case management and by providing support to families in crisis struggling with problems such as substance abuse and mental illness. Provide intensive, home-based case management and crisis intervention to help the family reach stability. Make referrals to partnership agencies and outreach to new agencies when required. Follow-up with service providers and program staff to monitor families progress. Assist parents and youth in learning behavior management techniques. Maintain progress notes and accurate case records into computer system with all necessary data entry in accordance with contract mandates. Transport and escort clients from appointments or to obtain emergency resources such as clothing or medication as needed in own vehicle when necessary. Participate and facilitate team meetings, case conferences, agency presentations and trainings. Assist in supervision of the Case Management and support staff. Serve as a resource to case managers in specialty areas of mental health and/or substance abuse. Assist with the training of new staff. Supervise interns and volunteers. Assign cases received by the Department of Probation to Case Managers in the Program Directors absence. Oversee the maintenance of agency vehicles. Facilitate and develop monthly educational workshops for adolescents when needed. Participate in monthly recreational events for the adolescents in the HB program when needed. Customarily and regularly exercises discretionary powers Hold weekly female and male adolescent groups when assigned by Director. Utilize the educational training, skills and experience obtained from advanced degree to provide appropriate professional services, techniques and strategies for clients.

 

 

Knowledge, Skills, & Abilities Required:

Masters Degree preferred, Bachelors degree required with at least 3 years of experience in the area of mental health or substance abuse treatment. Participation in at least 20 hours of training in the first six months of employment (with an additional 20 hours during the second six months of employment) on Homebuilders, mediation, working with adolescents, substance abuse, mental health, the juvenile and/or criminal justice system, and related topics. Training will be provided by outside trainers, EAC NETWORK and by partnering County agencies. Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments. Must have good communication skills, both oral and written. Must have access to automobile and possess valid New York State drivers license.
