Substance Abuse Program Coordinator -Days-Full Time LISW

Description:In accordance with the Mission, Core Values and Vision; the Substance Abuse Program Coordinator has complete responsibility for the assessment, planning, directing, controlling, and evaluation of all aspects of Substance Abuse Program and is responsible for the financial, human, quality, cost, and planning strategies to ensure effective, efficient, and viable services for the service line. Standardizes the delivery of services, focuses on service development, customer satisfaction and coordination with other system service lines. Collaborates with others in Diley Ridge Medical Center management to assure effective and quality services. Work in collaboration with nursing and Inpatient physicians in aspects of Substance Abuse to achieve strategic and operational objectives of the Substance Abuse Program. Liaison for the community networking agencies. Provide education and information regarding the service line to operating and clinical leadership.

Job Qualifications (Knowledge, Skills, and Abilities)

Education: MSW and certification as a clinical social workerLicense / Certification: LCSW required Current LISW license to practice as a social worker in the State of Ohio, Case Management Certification by ACMA or CMSA. Chemical Dependency certification or ACM/CCM preferred.Experience: 2-3-years of experience in discharge planning or crisis intervention in an acute care or ambulatory setting.Strong organizational skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements.Possess decision-making ability with results-oriented focus. Demonstrates strong initiative and ability to provide visionary leadership to move the service line forward.Possess excellent clinical assessment and analytical skills.Computer competencyPossesses effective interpersonal skills, critical thinking and decision-making skills, conflict management, can work across departmental boundaries, facilitates problem resolution in developing situations to complex organizational problems, and maintain a professional demeanor in difficult situations.Able to perform professional duties with minimal supervision.Ability to simultaneously manage and complete multiple assignments with accuracy and efficiency including in person and virtual consultations.Ability to work independently and exercise sound judgement in interactions with physicians, payers, colleagues and patients and their families in a team building, multidisciplinary environment.Demonstrated ability to work collaboratively with physicians to develop and advance programs and resolve problems.Ability to apply experience, analysis, intuition, and effective communication skills in maintaining and improving the quality of services provided by the service line.Knowledge of basic functional operations of utilization management, discharge planning, care coordination and transitions of care, and business office and third-party payers preferred.Maintains a professional demeanor.

JOB AND TECHNICAL COMPETENCIES

Managing Performance: Provides direction of the program with accountability for operations. Participates in direct patient care involving assessments, identification of substance abuse patterns, current health status including screening for withdrawal symptoms and underlying mental health conditions.Manages, as appropriate, the community, operational, and financial aspects of the Substance Abuse Program. Serves as a liaison to administration and represents the Substance Abuse Program on various hospital and community committees to enhance and foster optimal Substance Abuse Care including outside events.Financial Performance: Understands Diley Ridge Medical Center specific budgeting process. Demonstrates ability to create and monitor budgets, analyze/interpret budget and financial statements and information. Report on budget variance, performance improvement, and action planning process. Establish procedures to assure accurate charging mechanisms. Analyzes data to guide decisions regarding unit and department goal formulation, implementation, monitoring and evaluation.Professional Engagement/Shared Decision Making: Enables colleagues from all settings and roles to actively participate in organizational decision-making groups such as Leadership, Patient Quality and Safety, Department of Medicine, and Community Substance Abuse Committees and task forces, etc. Promotes professional competence and personal growth ensuring optimal patient care.Performance Improvement: Monitor clinical processes and outcomes and system issues related to the quality of care provided in collaboration with medical staff and Administrator; develop quality goals, audits, and case reviews; identify trends; and help outline remedial actions while maintaining confidentiality. Implements the key quality measures that have been identified for the system and the site. Ensures that comprehensive, quality data is disseminated to direct care nurses. Identifies significant findings and trends in overall patient satisfaction with substance abuse program as compared with bench marked sources.Communication: Clearly defines expectations as outcomes. Develop methods that allow for the flow of information and decision making to be bi-directional and horizontal between and among department staff, the leadership team, supporting social work/case management team and the CNO.Interdisciplinary Care Model Development: Ensures participation of colleagues at all levels in interdisciplinary activities to collaborate on policies and standards of care/patient education and utilize quality and process improvement. Works collaboratively with all disciplines including pharmacy, case management, physicians, and others as appropriate including community and outreach needs. Coordinate management across the continuum of substance abuse care, which includes the planning and implementation of clinical protocols and practice management guidelines, monitoring of in-hospital patients, and serving as a resource for clinical practice. Stabilize the complex network of the many disciplines that work in concert to provide high-quality care. Serve as an internal resource for staff in all departments and act as a liaison for outside agencies.Visibility and Accessibility: Collaborates with direct care nurses and physicians to improve the work environment in patient care. Utilizes various methods to be visible and accessible to patients, patient families and associates.
