NaphCare is hiring a Substance Abuse Discharge Planner to join our team of medical professionals to work at the Fulton County Jail located in Atlanta, GA.
As a Correctional Substance Abuse Discharge Planner, you will work alongside correctional officers and other medical professionals to assists incarcerated individuals with planning their transition back into the community. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs.
ResponsibilitiesThe Substance Abuse Discharge Planner is responsible for discharge planning for inmates with substance abuse issues being released from the facility. This involves assessing needs, developing discharge plans, coordinating care, and connecting individuals with necessary resources ensuring a smooth and successful transition to ongoing recovery and support. Duties and responsibilities may include and are not limited to:
Conducting thorough assessments of patients' social, emotional, medical, spiritual, and physical needs to understand their specific circumstancesDeveloping individualized discharge plans that address mental health and substance use needs, ensuring continuity of careConnecting patients with appropriate post-discharge resources, such as outpatient therapy, support groups, housing, and vocational rehabilitationWorking with healthcare teams, community resources, and other relevant parties to facilitate a seamless transition to the next level of careMaintaining accurate and up-to-date records of discharge plans, client progress, and referralsCollaborating with healthcare professionals, social workers, and community organizations to ensure seamless transitions and continuity of careMaintaining accurate and comprehensive records of patient interactions, discharge plans, and follow-up activitiesProviding information and support to inmates and their families regarding discharge planning, available resources, and post-release care optionsActing as a liaison between the correctional facility and community agencies to facilitate referrals and access to servicesAssist patients with a specific plan for medication managementAssist patients with the identification of a support network, instructions for future care and follow-upCoordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors QualificationsExperience / Requirements / Qualifications
Bachelor’s Degree in a human service field bachelor's degree in a related field like nursing, social work, psychology or healthcare Possess an active CPR certificationKnowledge in the development of discharge plans and coordination with community service providers Ability to maintain confidentiality of all proprietary and/or confidential informationExcellent communication skills are essential for interacting with patients, staff, and physiciansAble to work independently and in a teamAbility to analyze data and solve problems independently.Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirementsMust pass the facility’s criminal background check and drug screening
Physical Capabilities:
Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
