The Substance Abuse Case Manager provides Case Management services, as needed to patients under the care of assigned providers to provide frequent proactive contact to prevent relapse and promote stability, both regarding their opioid use disorder as well as in other parts of their lives. Ensure care coordination, effective use of resources and quality of care in accordance with regulatory agencies.


Essential Duties and ResponsibilitiesCompletion of initial orientation (Center wide and specific Emergency Medicine Department)Understand and put into practice hospital standards and division policies as it applies to employment at AMCH as well as the Department of Emergency MedicineProvide a single point of contact for multiple health and social services systemsAdvocate for our opioid use disorder (OUD) patients, assist with needs sometimes thought to be outside the realm of substance abuse treatment, and be flexible, community-based, and patient-orientedIntegrate social determinants of health into patients’ treatment plans and engage in a culturally competent manner.Use motivational interviewing techniques, strength-based counseling, behavioral activation, and proactive care navigationDistribute naloxone and provide overdose prevention education as part of harm reductionOvercome pharmacy and insurance barriers to optimize convenience for discharge prescriptions and directly schedule patients in clinics at patient preferred dates and times.Yearly participation in the Competency Based Evaluation ProcessProvide reminders to patients on important dates, such as scheduled follow up appointment visitsEngage patients that missed an addiction clinic appointment via Outreach ProtocolCreate a database by utilizing internet search engines and leveraging personal and work experiences to develop a list of organizations that often encounter individuals with OUDCreate and continually update a database of local medical and mental health providers who are accepting new patientsReach out to community organizations through email or telephone to inform them of our services and assess these organizations’ interests and needs.Coordinate transportation, connect patients with peer support and housing services, assist with job readiness, help with applications for state ID and social security cards, help apply or reapply for Medicaid, and help navigate DSS (Department of Social Services)Schedule medical and mental health appointments for patients who cannot for themselves. Provide reminders to those patients the day prior to appointment as well to ensure compliance.Travel between sites (Latham and Albany Medical Center) to attend outpatient suboxone clinicAll candidates must be fully vaccinated for COVID to be considered
QualificationsBachelor's Degree in social work, human services, sociology, criminal justice, counseling, psychology, or a related field from an accredited program is - required. - required Previous experience in the provision of case management for substance use disorders and/or counseling services in hospital, outpatient, or similar settings. - required Previous experience working with people who struggle with substance use disorders, have severe mental health conditions and/or experience homelessness. - required Experience using an electronic health record and working in a medical office. - required Excellent customer service skills preferred Equivalent combination of relevant education and experience may be substituted as appropriate.
