Concept Rehab is seeking a highly motivated Therapy Program Manager to oversee a sub-acute rehab program in Grand Rapids, MI. This full-time, non-exempt position plays a critical role in directing clinical care, ensuring regulatory compliance, and leading a team of passionate therapists.
As Therapy Program Manager, you will work closely with the Director of Operations to lead day-to-day operations, mentor staff, collaborate with the interdisciplinary team, and champion clinical excellence.
What You’ll Do:Lead daily operations of the therapy program and ensure the delivery of high-quality, patient-centered careDrive clinical programming, performance goals, and staff development
Guide staff in best practices for GG coding and PDPM reimbursement
Coordinate discharge planning and individualized care plans
Participate in facility meetings (UR, QA, Falls, Rehab, etc.) and staff scheduling
Oversee documentation accuracy, compliance audits, and EOM reporting
Support orientation of new hires, facility tours, and marketing initiatives
Report outcomes and key updates to the Director of Operations
What We’re Looking For:A strong clinical background with leadership experience in a skilled nursing/sub-acute setting
Deep understanding of PDPM, Medicare/Managed Care, and GG coding
Organized, proactive, and results-oriented leadership style
Excellent communication and collaboration skills
A high level of integrity and adaptability

Requirements· Hold a license to practice in the state(s) where services are being provided.
· Therapist with experience as a clinician, previous Rehab Manager experience preferred.
· Preferably have a minimum of one year of experience working in long-term care.
