We are looking for a dedicated Stroke Coordinator like you to be a part of our team. **Job Summary and Qualifications** As a Stroke Coordinator, you will coordinate all systems related to acute stroke care delivery. + You will monitor quality of care issues, and pathways for stroke care management including patient education.You will act as clinical liaison between physicians, clinical coordinators and administrative staff.You will provide outreach to EMS and the community at large in order to enhance recognition and treatment of acute stroke.You will assist with continued growth and development of Primary Stroke Center. You will facilitate, support and lead the stroke program committee. + You will coordinate care of the acute stroke patient (CVA), TIA (transient ischemic attack) patients across the continuum of care by collaborating with appropriate hospital departments and hospital and medical staff. + You will be responsible for researching best practices and clinical practice guidelines, and facilitating their inclusion in development/updates of CVA/TIA patient care protocols. + You will oversee the implementation and adherence to established treatment protocols and clinical trial protocols, ensuring regulatory compliance of the Primary Stroke Center. + You will assure continuous quality improvement through data collection, analysis, and process improvement using internal QI. + You will collaborate with the stroke team to develop standardized patient teaching materials. + You will collaborate with community partners and outreach to ensure community awareness of stroke signs and symptoms through education and health screening opportunities. + You will develop and implement, through the Education Department, a plan to meet identified education needs of employees involved in the acute stroke continuum of care. + You will serve as a resource to hospital and medical staff for baseline/functional measurements, stroke and treatment protocols, primary stroke center, current research, and best practice related to stroke. + You will be responsible for the reporting of stroke core measure abstraction as well as any other reporting activities required for the stroke. **What qualifications you will need:** + (RN) Registered Nurse + Associate Degree + BSN preferred + 3+ years of experience in hospital nursing + 2+ years of experience in units providing care to the neurological patient population + Experience as a clinical nurse coordinator of a stroke program preferred LewisGale Medical Center (https://lewisgale.com/locations/lewisgale-medical-center/) has provided quality healthcare services since 1909.
