Bon Secours seeks people that are committed to our values of compassion, human dignity, integrity, service and stewardship to create an environment where associates want to work and help communities thrive. **Spine Navigator - Registered Nurse - RN - Physician Office - Tuckahoe Orthopaedic & Physical Therapy - Richmond, VA** **Primary Function/General Purpose of Position** The Spine Nurse Navigator will facilitate the assessment, planning, education, coordination, documentation and evaluation of media and supportive nursing care provided to the Spine patients and their caregivers. The Spine Nurse Navigator works across facilities and practice groups in the Richmond market to facilitate care of complicated oncology patients. The Spine Nurse Navigator communicates with physicians, nurses, all departments within the hospital facilities, and community resources to expedite medically appropriate cost-effective care. The Spine Nurse Navigator will afford oncology patients the access to obtain efficient and comprehensive clinical care as well as the supportive services required in the care and surveillance of the patient before and during active treatment. **Essential Job Functions** Patient Coordination and Support Assesses spine patients and their caretakers from a holistic perspective to plan patient care. Facilitates the prompt and comprehensive implementation of services based on assessment and planning: (Needs may include physical, psychological, social, spiritual, educational and financial, personal, and communication issues of the patient, actual or perceived barriers impairing the comprehensive provision of the patient's care, prompt referral to services that address identified needs. ) Assists in the coordination and provision of supportive care such as support groups and family conferences with the treatment team. Provides education both verbally and in writing to patients and families regarding spine care and its treatment. Serves as a liaison between the patient and their physician(s) when indicated. Directly collaborates with medical team in the planning and delivery of care. Works as a member of the patient treatment planning team to ensure that the holistic needs of the patient including the physical, mental, social, communication, financial, or information barriers do not impair the provision of the patient's comprehensive care plan. Demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patient served in his or her population and across the continuum of care. Demonstrates knowledge of principles of growth and development of the life span and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his or her age, specific needs and to provide the care needed. Develops and maintains collaborative relationships with various members of the Spine team, i.e. physicians, nurses, social workers, and laboratory/radiology personnel, etc. Develops relationships with the various community based support institutions which would assist in the care and support of the Spine patients of the Bon Secours Health System, i.e. volunteer organizations, churches, etc. Bridges ambulatory and inpatient spine services through open and frequent communication. Develops and participates in a method to track key quality outcomes such as patient satisfaction, physician satisfaction, participation in clinical trials, and activities to enhance revenue. Participates in Bon Secours Health System sponsored community awareness and educational functions, as related to the Spine program. Run reports, analyze data, plans intervention and programmatic changes based on data analysis. **Licensing/Certification** . This position requires an RN degree and a current Commonwealth of Virginia Nursing License. **Education** Baccalaureate' degree required **Work Experience** At least 2 years nursing experience in community nursing, critical care or emergency department experience preferred. Spine experience strongly preferred. **Training** Proficiency in basic computer skills, including the ability to maneuver within a Microsoft Windows environment.
