Speech Language Pathologist (PRN)
**Employment Type:** Part time **Shift:** **Description:** **Speech Language Pathologist – PACE Program** Join our mission-driven team at Trinity Health PACE, where you’ll play a vital role in enhancing the lives of older adults. As a Speech Language Pathologist, you will evaluate and treat participants with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. Your work will support participants in achieving their highest level of independence and quality of life. **Key Responsibilities:** + Provide assessments and individualized treatment plans following Interdisciplinary Team approval. + Recommend safe swallowing strategies and appropriate diets. + Document care in TruChart and maintain accurate records. + Collaborate with team members to develop participant-centered care plans. + Ensure compliance with all regulatory and ethical standards. + Participate in training, team meetings, and quality improvement initiatives. **Qualifications:** + Master’s degree in Speech Language Pathology or related field. + Current, active, and unencumbered SLP license in the state of practice. + ASHA certification and CPR certification required. + Strong communication, organizational, and interpersonal skills. + Proficiency in Microsoft Office and electronic documentation systems.
