Key Responsibilities:

Works closely with providers to assess patients' and families' medical and psychosocial needs and assist them in follow up care and management. Develop and implement individualized care plans. Coordinate medical services and resources. Monitor and evaluate patient progress.

Requirements:

Education: Bachelor's degree preferred Licensure: BSN or LCSW preferred Experience: 3 to 5 years of experience
