Position Purpose
As a Social Worker PRN at St. Mary’s Hospital, you will provide compassionate, patient-centered care by delivering direct psychosocial support to patients and families. You’ll collaborate with physicians, hospital staff, and community agencies to facilitate effective discharge planning and ensure continuity of care. Your approach will be tailored to meet the age-specific needs of patients ranging from children to older adults.
What You Will Do
Provide direct social work services to patients and families in a hospital setting.Assess psychosocial needs and develop individualized care plans.Collaborate with interdisciplinary teams to support discharge planning and resource coordination.Communicate with community agencies to ensure appropriate post-discharge support.Adapt care strategies based on the developmental and emotional needs of diverse age groups.Minimum Qualifications
MUST commit to minimum of two weekend shifts per month AND a minimum of two Holiday shifts per year.Education:Bachelor’s degree in Social Work or related field required.Master’s degree in Social Work (MSW) preferred.Experience:Minimum of 1 year experience in a hospital or healthcare facility.Strong interpersonal and communication skills.Skills:Ability to work independently and collaboratively within a team.Strong clinical knowledge base.Demonstrated problem-solving, critical thinking, and time management skills.Position Highlights
