 Territory: Northern Chicago – Maywood, Oak Park, Forest Park, Melrose Park, Elmwood Park, Franklin Park, Schiller Park, Schaumburg, Carol Stream, and Chicago ZIP codes north of North Ave.

Join Loyola Medicine Home Care and Hospice, a member of Trinity Health At Home, as a  Master Social Worker (MSW). In this meaningful role, you’ll provide compassionate, one-on-one care to patients in their homes, helping them navigate emotional, psychological, and safety challenges during their care journey.

What You’ll Do

Conduct initial assessments to identify patient needsDevelop and implement individualized care plans in collaboration with physicians and interdisciplinary teamsConnect patients and families with appropriate community resourcesEnsure compliance with federal, state, and local regulationsParticipate in quality improvement initiatives

Qualifications

Master’s Degree in Social Work (MSW)Licensed Social Worker in the State of IllinoisMinimum 1 year of experience in a healthcare setting (home care/hospice preferred)
