Join Loyola Medicine Home Care and Hospice, a member of Trinity Health At Home, as a Master Social Worker (MSW). In this meaningful role, you’ll provide compassionate, one-on-one care to patients in their homes, helping them navigate emotional, psychological, and safety challenges during their care journey.

Master’s Degree in Social Work (MSW)Licensed Social Worker in the State of IllinoisMinimum 1 year of experience in a healthcare setting (home care/hospice preferred)Benefits That Support You
