SOCIAL WORKER - Oncology

Provide individual, family, and group counseling focused on coping with diagnosis, treatment, grief, and survivorship.

Facilitate communication between patients, families, and healthcare providers to ensure patient-centered care.

Assist Medical Oncology Manager and Specialty Pharmacist with Oral Chemotherapy Assistance Programs

Identify and coordinate access to community resources, financial assistance, transportation, housing, and other support services.

Advocate for patient needs within the healthcare system and with outside agencies.

Assist Financial Navigator with Patient Assistance Programs, Grants, and Foundations

Lead or participate in support groups for patients and caregivers.

Maintain accurate documentation in compliance with hospital, state, and federal regulations.

Participate in interdisciplinary team meetings, case reviews, and discharge planning.

Provide crisis intervention as needed.

Stay current on developments in oncology care, psychosocial oncology, and related social work practices.

 

MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

Bachelor's (BSW) or Master's (MSW) degree in Social Work from accredited school preferred.

Social worker with at least two (2) years experience in clinical medical case management in acute care, extended care or rehabilitation preferred.

Current BLS Healthcare Provider card.
