Job Overview:
The Social Worker provides evaluative and inpatient mental health services to the patients/families of the inpatient unit. Maintains close contact with community providers serving patients. Prepares and maintains records and reports regarding patient treatment and progress as required by regulatory standards and organizational policy. Participates as an integral part of the Milieu to provide a safe and secure treatment environment. Participates in the unit PI/QA programs and hospital quality improvement efforts. Is responsible for reporting identified safety issues such as hazardous environments i,e,damaged floors/walls/ceiling tiles/unsecured areas and medical errors, etc.
Qualifications:
Required:
+ Master's Degree in Social Work
+ LSW or LCSW license required
+ 3 years experience/training working in a mental health setting
+ Experience/training in individual, group and family therapy
+ Knowledge of behavioral therapy concepts
Scheduling Requirements:
+ Full-Time
+ Day Shift
+ 40 hours per week
Essential Functions:
+ Functions as a member of the multidisciplinary treatment team to formulate individualized treatment plans through establishing patient treatment needs and methods of addressing these needs. Areas addressed in the treatment plan shall include: Observed psychopathology, psychosocial/family assessments, legal status, daily functioning, available community supports, educational status and the accurate and timely assessment of current mental status.
+ Performs evaluations of patients and their families according to the time frames established through unit policies and procedures. Performs direct psychotherapy treatment services, such as: Individual, group and family therapies according to established goals and objectives of individual patient treatment plans. Functions as a liaison with community providers to maintain productive relationships
+ Prepares and maintains accurate records and reports as they related to patient assessment, treatment and progress, court hearing and case reviews, inclusive of USTF forms for State. Documentation must adhere to regulations set forth by all regulatory, organizational and official agencies.
+ Serves as a member of the unit s quality assurance teams.
