Job Overview:
The Social Worker Licensed designs and implements treatment plans, including individual, group and/or family psychotherapy and evaluates and documents patients outcomes according to departmental policy and procedures.

Interprets nature of illness and prescribed course of treatment to patients and familiesCommunicates family issues to other staff membersParticipates in planning conferencesIdentifies substance abuse/use issues and incorporates into tx plan identifying triggers, support mechanisms and 12 step participation if appropriateParticipates in supervision activities, as directedMaintains up-to-date and complete records of patient information according to established standardsDocuments progress notes within 24 hours of DOSUnderstands the payer identification process and ensures that services delivered are accurately authorizedAs a member of the multidisciplinary team, assists in evening and updating treatment planInvolves families/referral sources in all treatment endeavors unless contraindicatedAdheres to department s practice guidelinesMaintains current knowledge of professional practice standardsMaintains current knowledge of relevant addictions treatment and researchPerforms random UDS on patientsIn collaboration with treatment team, implements the discharge planCollaborates with other Hospital programs and community agencies to promote coordination and continuity of patient and family carePerforms the psychosocial assessment which includes family dynamics and treatment needs according to specified outpatient/inpatient standardsBenefits and Perks:
