This is for a per diem Home Health Social Worker.
Position Summary: Responsible for conducting psycho‐social interviews with patients of all ages and their families in order to aid patients and families with personal and environmental difficulties which may predispose illness or interfere with obtaining maximum benefits from medical care. Follows agency policies and procedure.
Education, License & Cert: Masters’ Degree Required.
Experience: Must be able to work effectively and professionally with a wide cross‐section of individuals. Must have high ethical standards with respect to discretion and regard for confidential information. One year social work experience in a health care setting.
Essential Functions: 1. Recognizes and provides for the psyhco‐social needs of patients of all ages and their families. 2. Evaluates and addresses (treatment) crises situations and disability resulting from the emotional, social, and economic stresses of illness. 3. Provides assistance to medical, nursing, and other health care personnel in arranging for prescribed medical, i.e., psychiatric alternative treatment and participates in discharge planning functions as needed. 4. Arranges for specialized assistance from community resources for patients of all ages and families who need such assistance. 5. Counsels patients and families to deal with the particular stresses affecting them. 6. Follows patient safety and infection control policies and procedures. 7. Assists patients and their families in areas of financial matters related to hospitalization. 8. Informs and updates the Case Manager regarding complex problems of case clientele. 9. Maintains good working rapport and cooperative relationships with other disciplines on the team.

The range for this position is $29.04-$37.41-$45.78.
