Social Worker - Disability Services

### Job Duties
Under supervision of a Program Manager or Supervisor, and within Federal, State, and locally developed programs and policies, supports individuals diagnosed with a disability. Identifies individual needs and county and community support services to address needs. The services help an individual live independently in community settings and promote optimal health, safety, and community integration. Will be working within a multi-disciplinary team setting. Supports a diverse, respectful, and inclusive workplace.
### Minimum Education Required
A bachelor's degree from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.
OR
A bachelor's degree from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
### Minimum Experience Required
A bachelor's degree from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.

A bachelor's degree from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
