As a Social Worker on our team, you will utilize your specialized knowledge in crisis intervention, resource seeking, mediation, counseling and empowerment to care for patients of all ages, backgrounds and needs. Your work will directly impact their lives as you work to help them improve their overall well-being.
A caregiver in this position works days, 8:00am - 5:00pm.
A caregiver who excels in this role will:
Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings).
Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
Maintain a network of resources for consultation and connect patients with community resources.
Supervise Social Work Interns and assist with training new caregivers.
Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
Minimum qualifications for the ideal future caregiver include:
Master’s Degree in Social Work from a CSWE accredited program
Current state licensure as a Licensed Social Worker (LSW)
Two years of experience in a hospital or healthcare setting
State licensure as a Licensed Independent Social Worker (LISW) within five years of hire

Physical Requirements:
Manual dexterity to operate office equipment.
May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
Must have normal or correction vision
Must be able to clearly communicate verbally by phone.
