**Employment Type:** Full time **Shift:** Day Shift **Description:** Social Workers in this position work on a team with Nurses and Community Health Workers to case manage Medicaid eligible pregnant women and infants up to age one and their families. The social worker offers support, education, systems navigation guidance, linkage to community resources, and encouragement to nurture health and wellness throughout pregnancy. The social worker (as a member of the home visiting RN/BSW dyad) will visit the expectant mother in their home and for up to 12 months after the child is born. + **Job Title:** Social Worker **Location:** Clinica Santa Maria **Job Summary** We are seeking a compassionate and skilled **Social Worker** to join our team in delivering patient-centered care across home and clinical settings. This role requires independent judgment in planning, coordinating, and evaluating care, while working within established policies and procedures.
