**Description:**
Social Workers in this position work on a team with Nurses and Community Health Workers to case manage Medicaid eligible pregnant women and infants up to age one and their families. The social worker offers support, education, systems navigation guidance, linkage to community resources, and encouragement to nurture health and wellness throughout pregnancy. The social worker (as a member of the home visiting RN/BSW dyad) will visit the expectant mother in their home and for up to 12 months after the child is born.

**Job Summary**
We are seeking a compassionate and skilled **Social Worker** to join our team in delivering patient-centered care across home and clinical settings. This role requires independent judgment in planning, coordinating, and evaluating care, while working within established policies and procedures. The Social Worker will conduct assessments, develop care plans, and support patients in overcoming barriers to care. _Note: This position does not involve clinical social work practice such as diagnosis or psychotherapy._
**Key Responsibilities**
+ Conduct psychosocial assessments of patients, families, and support systems
+ Develop and implement strengths-based care plans
+ Provide brief, solution-focused counseling and patient advocacy
+ Facilitate referrals to community resources and coordinate continuity of care
+ Conduct one-on-one home or office visits (minimum 30 minutes)
+ Educate patients and families on health topics and available services
+ Collaborate with interdisciplinary teams and Medicaid Health Plans
+ Accurately document assessments, plans, interventions, and outcomes
+ Promote positive decision-making, stress management, and coping skills
+ Ensure compliance with applicable policies, guidelines, and laws
+ Uphold the NASW Code of Ethics
+ Perform other related duties as assigned

+ Knowledge of psychosocial interventions and community resources
+ **Preferred:** Fluency in Spanish
+ **Experience**
+ **Required:** Minimum 1 year of social work experience with families or equivalent education/experience
+ **Preferred:** 2+ years in a healthcare setting; experience in community health, case management, and patient advocacy
+ **Preferred:** LMSW (Licensed Master’s Social Worker) with clinical designation **Licensure**
+ **Required:** Current Michigan licensure as a Licensed Social Worker

+ **Required:** Bachelor’s Degree in Social Work (BSW)
+ **Preferred:** Master’s Degree in Social Work (MSW) from an accredited institution
