**Overview** **We are looking for a Remote Licensed MSW to join our team, covering Grand Rapids, MI.** Join a mission-driven team that is redefining how care is delivered to patients facing serious illness. As a Social Worker in our Advanced Illness Management (AIM) program, you will provide psychosocial support, guide patients and families through complex care decisions, and help navigate the emotional, spiritual, and social needs that accompany serious illness.
