Job Title: Licensed Master of Social WorkDepartment: Social Work Location: 20 Cottage Street - Potsdam Hours Per Week: 37.5Schedule: Monday-Friday, 8AM-4PM or 9AM-5PM with one weekend rotation every 10 weeks SUMMARY: Perform a comprehensive list of psychotherapy functions and practices. Work with internal and external stake holders to promote quality patient- centered behavioral health care. This is a 17-bed in-patient rehabilitation unit in which the LMSW would serve as a primary counselor with a small caseload, which in turn provides you the opportunity to spend more time in 1:1 sessions. RESPONSIBILITIES: + Identifies potential barriers and current difficulties in participating in treatment at intake and throughout the course of treatment. + Appropriately utilizes screening and assessment tools approved by CPH including but not limited to CSSR-S, PHQ-9, Modified mini, substance use and risk assessment. + Responsible for an individual case load including 1:1 counseling, treatment, relapse prevention, discharge planning, care coordination and referral + Provide individualized patient care utilizing best practice counseling skills, techniques and modalities to improve quality of life and reduce symptoms + Engage in ongoing professional maintenance activities to include but not limited to clinical supervision. continuing education, and counselor wellness + Development, execution and documentation of group therapy on assigned topics to be determined by patient need and counselor skill set + Provide psychoeducation to individuals, groups and families on topics such as substance use disorders, mental health and wellness, life style changes, and coping skills, and recovery. + Documenting: In a variety of manners including assessments, individual and group notes, collaboration, collateral contact with internal and external stakeholders to the patient. + Treatment planning, discharge planning and regularly updating the patient record to demonstrate patient’s progress or lack thereof in the treatment environment. + Provide clinical supervision for interns and or junior staff of the same discipline + Act as a mentor, coach and evaluator to create an atmosphere that promotes counselor self- reflection, discipline, motivation, learning, wellness and professional development. REQUIRED QUALIFICATIONS: + Required: Direct client experience in a human service setting + Required Education: Master’s Degree in Mental Health Counseling or Social Work + Required Certification: LMHC or LMSW PREFERRED QUALIFICATIONS: + Preferred: Individual and/or group counseling experience EDUCATION: LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. It may be modified in the future and eligible for additional pay components.
