Aspirus Wausau Hospital in Wausau, WI is seeking a SOCIAL WORKER to join our CARE COORDINATION team!In this role you will assist patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. Works with patients and families to obtain psychosocial history, assist patients in resolving environmental difficulties interfering with attainment of maximum benefits from medical care, and conduct routine discharge planning. Directs patients to designated community agencies or resources, as necessary. Works with clinical social work staff to foster collaborative practice that will optimize use of resources and assure customer satisfaction. Accountable for evaluations, supervision, leadership, and fiscal outcomes of the Social Work team members.
