Summary:
The HUP social needs response team is looking for a full-time social worker to join the team!
Responsibilities:
• Provide support via telephone to patients referred to the Social Needs Response Team (SNRT) for assistance with health-related social needs (HRSN)
• Provide onsite support to patients in crisis and patients with positive HRSN assessments
• Make follow up calls to recently discharged patients to provide support with issues impacting length of stay, ED visits, and readmissions
• Connect patients with appropriate resources
• Assist patients with completion of Medicaid applications
• Assist with onboarding of SNRT student interns
• Facilitate student education to advance understanding of SNRT goals and resource connection
• Review student documentation in real time and provide feedback to students
• Interact with students to provide ongoing support as students work out complex needs
• Collaborate with SNRT leadership to help define and track metrics of success
Credentials:
+ Licensed Social Worker (preferred).
+ MSW (required).
Education or Equivalent Experience:
+ Education Specialization: MSW (required)
+ Experience: 3+ years Behavioral health experience in the field, health care setting experience helpful, direct practice experience helpful.
