Social Care Network Case Manager(Full Time/Monday-Friday Days)

This position is responsible for the day-to-day operations of Caz Recovery’s service provision through the WNY Integrated Care Collaborative (WNYICC), which is WNY’s regional Social Care Network (SCN). This position is responsible for all aspects of screening, navigation, care management, and housing services related to the SCN service contract. Funding for this position is contingent upon NYS’s Health Equity Reform 1115 Waiver and is expected to expire in spring 2027.

Duties and Responsibilities

Provides empathetic and culturally competent screening for social determinants of health to potential program participants telephonically, virtually, and/or in-personProcesses screenings and referrals daily including calls with potential program participants, completes documentation and insurance/eligibility verificationsReceives, prioritizes, monitors, and triages screenings and referrals that may require connections across external community-based organizationsManages incoming referrals received and ensures successful and timely connections are made between applicants and appropriate community-based organizationsDocuments progress and actions taken with each referral and every interaction in the WNY Integrated Care IT platform, as requiredConducts in-home assessments with community members when necessaryMaintains effective communication with potential program participants and/or referring organizations to ensure acceptance and resolution of referralsAssists program participants in searching for, obtaining housing, and accessing relevant services and benefits including furnishing and initial pantry stockingAssists program participants with coordinating the “moving in” processFacilitates relationships between program participants and landlords/property managers, advocating for program participants when necessaryEstablishes working relationships with landlords/property managers and maintains a roster of available housing unitsWorks in collaboration with program participants to create individual service plans to support participants’ growth, empowerment, and self-sufficiencyInforms appropriate staff of any concerns or changes in program participants’ demeanor that may signify exacerbated mental health or other concerns that require intervention.Visits program participants’ homes at least once monthly to inspect the unit and ensure participants’ safety and security in the homeAssists program participants in securing linkages to social supports that lead to successful community integrationAssists program participants in achieving financial independence by obtaining income, creating and following budgets, and participating in educational or vocational programsResponds to and manages crisis or emergency situations that may ariseAccurately completes case notes in a timely mannerManages program participant’s housing chart and ensures all necessary regulatory documentation is currentAttends WNY Integrated Care Screening/Navigation and Housing workgroup meetingsWorks within WNY Integrated Care social care technology and transportation platformsParticipates in weekly staff meetings and case conferencesCarries out duties essential to the above stated responsibilities as assignedAttends trainings as assigned to increase knowledge of homelessness and substance useParticipates in community wide meetings to build a network of other support service workers and to increase knowledge of local resourcesIs mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, program participants, and external partnersPractices Housing First and Harm Reduction in all duties and interactions with program participantsRegular attendance at work locationMaintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.Other duties may be assignedReasonable accommodation may be made to enable individuals with disabilities to perform the essential functions


Requirements Master’s degree in human services or related field ORBachelor’s degree in human services or related field and a minimum of 6 months’ human services experience ORAssociate’s degree in human services or related field and a minimum of one-year human services experienceIn addition, the qualifications above, a valid New York State driver’s license is required and a reliable automobile Salary Description $20.00/Hour
