SkillSpring Workforce Programs Case Coordinator
The SkillSpring Department  of The New Jewish Home is looking for a full time Workforce Programs Case Coordinator.   JOB SUMMARY: As a key member of the SkillSpring (f/k/a GCD) department, the Workforce Programs Case Coordinator will engage with current participants in the young adult and high school programs in an effort to prepare for and secure employment.  The role of the Workforce Programs Care Coordinator is to collaborate with SkillSpring participants using the care management process including: assessing a person’s functional level, their physical, cognitive, social-emotional status and their support system; developing a plan of care that addresses the needs and problems identified and incorporating the services that are needed to enhance the current support system; identifying and arranging for coordinated delivery of those services; monitoring changes in the person’s condition, and reassessing the person’s needs on a regular basis. This team member will serve in a coaching role and effectively advocate for program participants to have their various needs met with compassion and grace.  The Workforce Programs Case Coordinator must possess a strong work ethic, a passion for working with underserved young adults, and be able to build and manage relationships with internal and external partners contributing to the professional success of the SkillSpring Department and its program participants.  REPORTS TO:     Young Adult Program Manager- Workforce Programs DUTIES AND RESPONSIBILITIES: 1.     Ensure the health, safety and well-being of participants in the young adult program, by providing support and supervision of all activities. 2.     Screen and identify young adult participants in need of care coordination services. Conduct comprehensive assessments by interviewing young adult participants to identify their participant’s needs and goals. 3.     Collaborate with the SkillSpring Young Adult or Health Care Explorers teams, to identify problems or needs that require special planning, intervention, teaching or follow-up. 4.     Actively support measures that prioritize the participant’s needs and promotes the effective use of resources. Facilitate the participant’s involvement in the direction of care. 5.     Identify, plan and arrange for appropriate services by applying a knowledge of local regional and federal services and regulations. Collaborate with the participants, programming teams and community services. 6.     Coordinate implementation of the care plan with case conferences and working collaboratively with community based partners on these conferences. to maximize continuity of care 7.     Monitor the participant’s needs and responsiveness to care plan interventions. Communicate with providers who are delivering care and services for the purpose of maintaining the quality of care. Maintain and revise care plan as the participant’s needs change. 8.     Document assessments, plans, interventions, the participant’s goals and their response to care management. Evaluate the participant’s response to the plan of care and the appropriateness of services. Collaborate with team members to identify the participant status and need for care plan adaptation. 9.     Participate in process improvement groups. Utilize benchmarking and other sources to identify best practice. Participate in setting performance targets to improve quality. Promote and assist with development of appropriate education materials. Promote and facilitate effective communication throughout the continuum of care. 10.  Maintain, update and actively improve care management skills, including identification of high risk individuals, care coordination, transition planning and implementation and knowledge of community resources. Work collaboratively with SkillSpring staff. 11.  In collaboration with the Workforce team, facilitation of young adult programming including professional development, clinical skills training, and health career exploration. 12.  In collaboration with the Workforce team, monitor participation of young adult programming by scheduling, tracking and reporting on various data points, including but not limited to attendance and surveys. 13.  In collaboration with the Workforce team, support the SkillSpring team, leading and supporting other program activities as needed. 14.  Create an environment that is diverse, culturally sensitive and relevant to the needs of young adults. 15.  Perform other related duties as required. *This position may require some mornings, evenings and Saturdays at multiple sites. Hours, days and locations fluctuate based on seasonal changes.
