Status: Full-Time | In-Office with Travel Required
Department: Private Duty & Consumer Directed Services (CDS)

This role plays a vital part in supporting clients and caregivers within our Private Duty Care and Consumer Directed Services (CDS) programs.

What You’ll Do:
+ Serve as a professional representative of Phoenix with clients, caregivers, and the community
+ Manage and process intake paperwork for new Private Duty Care clients
+ Coordinate start-of-care logistics in collaboration with the Care Coordinator
+ Support ongoing monthly case management duties for CDS
+ Assist with electronic visit verification (EVV) compliance and visit accuracy
+ Maintain up-to-date, clear, and accurate client records
+ Conduct frequent in-home visits with clients across multiple counties
+ Prepare and submit timely documentation and reports per policy guidelines
+ Participate in community outreach and marketing events and initiatives
+ Support general clerical tasks and assist with resolving client or caregiver concerns
+ Attend in-services, staff meetings, and quality assurance activities
What You Bring:
+ Minimum age: 21 years
+ High school diploma or equivalent required
+ Supervisory experience preferred
+ Experience in an office setting required
+ Ability to work independently while staying organized and detail-oriented
+ Strong communication skills with the ability to conduct client interviews professionally
+ Ability to manage multiple tasks in a fast-paced environment
+ Previous home care experience is highly preferred
Physical Requirements:
+ Frequent sitting, standing, walking, and manual dexterity
+ Occasional lifting up to 50 lbs and pushing/pulling over 100 lbs
+ Occasional reaching above shoulder level
+ Must be able to drive for business-related travel
