**SUMMARY:**
Under the supervision of the Manager of Quality and Sepsis and in conjunction with Nursing Leadership, the SAHS Sepsis Coordinator identifies and analyzes the care of the septic patient at SAHS. Works with medical staff to provide real-time education for colleagues for the treatment of sepsis with the end goal of improving quality of care.
Engages and interacts with medical staff, resident physicians, allied health professionals, and nursing staff to meet the necessary objectives of standards of care. The coordinator may be asked to participate in other quality initiatives and projects that fall outside the scope of sepsis if necessary, based on the needs of the organization.
**HOURS** : Office location can be at one of the Saint Alphonsus hospitals. The Sepsis Coordinator will consult in person in the Boise and Nampa hospitals regularly. In-person consultation and meetings with the Baker City and Ontario, Oregon hospitals will be twice monthly. The selected Coordinator will be able to work remotely about once a week. The selected Coordinator will work traditional, daytime hours with some flexibility in start/end times.
**REQUIREMENTS** :
Must be licensed in the State of Idaho as a Registered Nurse as defined by the Idaho State Board of Nursing at the time of hire. An Oregon Nurse license is required within 90 days of hire.
Bachelor’s degree in Nursing required.
Experience as a program coordinator, in a nursing leadership position or as an educator preferred.
At hire, Basic Life Support certification from the American Heart Association or American Red Cross is required. (ACLS and/or PALS may be accepted in lieu of BLS) Other certifications may be required as identified in the SAHS Certification Crosswalk
**ESSENTIAL FUNCTIONS** :
Utilize advanced theoretical and empirical knowledge of physiology, pathophysiology, pharmacology, and health assessment to improve clinical care. Assess and intervene in complex health care problems within sepsis, and select, use, and/or evaluate technology, products, and devices appropriate to the specialty area of practice.
Provide clinical expertise and make recommendations to physicians, nursing, and other healthcare team members and patients/families. Utilize evidence-based clinical practice to develop methods to improve patient care and patient care outcomes.
Evaluate policy and procedures for clinical practice of sepsis. Assess, plan, implement, and evaluate educational programs to meet the identified needs of the staff, patients, family/significant others, and community through various teaching modalities.
Provide mentorship and/or coaching to staff, students, and others to encourage staff development. Utilize performance improvement methodologies as an avenue to improve advanced clinical practice and care in specialty area. Co-Chair the Sepsis Multidisciplinary Committee and assist the team in defining direction, goals and action plans to continue to improve early identification of patients, and compliance with interventions and outcomes (create agendas, finalize minutes and ensure follow up).
Serve on unit/department specific Sepsis Committees and work groups as needed. Work with other system sepsis leads developing the sepsis program through Trinity Sepsis Collaborative.
Facilitate the implementation and evaluation of the sepsis program including all systems necessary for the multidisciplinary approach throughout the continuum of care. Coordinate the Sepsis Multidisciplinary Committee to facilitate outcome measures for the sepsis program including:
Improvement in compliance with:
+ Surviving Sepsis Campaign Resuscitation
+ Management Bundle, Reduction in length of stay (LOS)
+ Reduction in mortality rates
+ Reduction in sepsis related readmissions
The Sepsis Coordinator will collaborate with:
+ Informatics by supporting the development of the electronic medical record (EMR) such as RN screening tools, MD Order sets, MD documentation, and the identification of additional opportunities.
+ Physician partners to determine baseline and ongoing compliance with physician documentation and compliance with the Sepsis bundle.
+ Clinical documentation specialists to drive appropriate documentation to optimize coding.
+ Frontline staff to identify on-going care concerns related to sepsis care and works with them to develop strategies for improvement.
+ Leadership and colleagues in identifying sepsis quality of care issues, in order to reach collaborative action plans and outcome evaluations.
+ Nursing management, nursing education, physician leadership, medical staff and marketing department to facilitate program projects and sepsis education, including formal and informal education to medical and clinical staff regarding early identification and management of patients with severe/septic shock. This includes new staff orientation for all nurses.
+ Quality department to provide oversight and support for the core measure data abstractors related to the sepsis measures.
+ House-wide Sepsis Committee, Quality Department, physicians and nursing leadership to facilitate the sepsis program projects and initiatives.
+ Emergency department (ED), Intensive Care Unit (ICU), Med/Surg (MS), Women & Infant Services (WIS) and other departments.
Utilize currently available reports to identify sepsis cases and facilitate collection process; assess and analyze outcomes. Recommend additional reports as necessary. Analyze data to identify trends and issues; utilize improvement tools to assist with problem solving and action planning. Present data to Quality and other hospital and medical staff committees.
