Summary This position is in the Mental Health Service located at the Bedford VA Healthcare System. This position is full-time at 40 hours per week. Responsibilities This position provides case management and intervention support to high risk Veterans, in conjunction with the Suicide Prevention Program in a team oriented, recovery-based program. The (SUD) Specialist supports the Suicide Prevention Program by providing continuity of care to high risk Veterans with SUDs as they transition from programs such as Domiciliary or Residential Rehabilitation and Treatment Programs (RRTPs), Inpatient hospitalization, outpatient mental health, Community Residential Programs, Health Care for Homeless Veterans Contracted Housing, Grant Per Diem Programs and all transitions. The SPC-SUD provides on-going substance use case management and early treatment interventions to promote sustained stability and abstinence to promote recovery and stabilization. The SPC-SUD must be able to develop appropriate professional relationships with the Veterans and meet the Veteran where he or she is ready to engage in treatment. The SPC-SUD also will need to develop liaison relationships with more acute treatment providers/ programs, such as an Intensive Outpatient Program. The SPC-SUD will provide appropriate group and individual case management and treatment. Duties may include, but are not limited to: Provides direct SUD services for high risk Veterans in the Medical Center. Provides Outreach services for high risk Veterans in the community. Provides training for other members of the multidisciplinary treatment team in areas related to SUD assessment, anger management, and life skills and SUD with co-occurring disorders. Independently conducts risk assessment of Veterans. Identifies symptoms leading to clinical diagnoses of SUD and high risk behaviors. Provides all necessary assessment functions, e.g. interviewing, psychosocial histories, and SUD assessments to aid in the development of treatment plans. Develops comprehensive treatment plans with the Veteran that address the factors that contribute to high risk status, including SUD and other co-occurring factors within a recovery model framework. Works with the Veteran to realize the Veteran's treatment goals, and reassesses and revises goals as indicated by the Veteran's engagement in treatment. Organizes and conducts individual and group case management and SUD treatment, to include early interventions and supportive follow up to ensure stability. Provides SUD support to the Suicide Prevention team as a functional member of this team, to include consultation, interventions, or accompanying other team members on assessments throughout the facility. Serves as a member of the Suicide Prevention team. Develops partnerships with community agencies, especially those serving high risk and SUD Veterans, with regular contact and communication. Provides consultative services to Suicide Prevention and all areas that serve high risk Veteran and provide supervision for dependent level staff and trainees. Participates in policy making, goal setting, and treatment service planning discussions with other members of Suicide Prevention Program. Provides appropriate clinical documentation for all contacts with or about Veterans. Responsible for the collection of statistical data and for reviewing the data to improve performance and delivery of high risk Veterans with SUD. Provides recovery -oriented treatment with the goal of establishing the Veteran independently in the community at the Veteran's highest level of functioning and quality of life. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Grade Determinations: GS-12 Senior Social Worker: Experience and Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
