Humana Healthy Horizons is seeking a Senior Program Delivery Professional to support quality program work in multiple states. They will be responsible for implementing strategies to improve the quality of care, health outcomes, member experience, or service delivery model related to Long-Term Services and Supports (LTSS) and Home and Community-Based Services (HCBS). The Senior Program Delivery Professional (LTSS - Quality) works on problems of diverse scope and moderate complexity.
The Senior Program Delivery Professional develops insights and strategies to support high quality LTSS service delivery.
+ Plans, organizes, monitors, and oversees complex projects with cross functional teams to deliver defined requirements and meet company strategic objectives.
+ Utilizes data to identify, develop, and implement evidence-based quality improvement and process improvement strategies.
+ Coordinates with cross-departmental teams to integrate quality improvement processes, rapid-cycle improvement and activities.
+ Maintains detailed project documentation including meeting minutes, action items, issues lists, project plans, and risk management plans.
+ Negotiates with project stakeholders to identify resources, resolve issues, and mitigate risks.
+ Provides functional and technical knowledge of LTSS and quality improvement principles in order to support Humana Healthy Horizon’s quality strategy and implementation approach.
+ Works creatively and analytically in a problem-solving environment demonstrating teamwork, innovation and excellence.
+ Provides status reporting regarding project milestones, deliverables, dependencies, risks and issues, communicating across leadership.
+ Understands interdependence between technology, operations and business needs.

**Required Qualifications**
+ Bachelor's degree.
+ Three (3) years of experience in healthcare Quality Improvement.
+ Previous experience in LTSS quality measures (example HEDIS).
+ Previous experience working with Medicaid populations.
+ Understanding of healthcare quality improvement strategies such as Institute for Healthcare Improvements (IHI) Model for Improvement.
+ Comprehensive knowledge of Microsoft Office Outlook, Excel, Word and PowerPoint.
+ Excellent communication skills, both oral and written
+ Ability to present to large groups.
**Preferred Qualifications**
+ Master’s degree in nursing, public health, social work, or another relevant field.
+ Certified Professional in Health Care Quality (CPHQ).
+ Experience working in a LTSS or HCBS quality role or service delivery setting.
A **dditional Information**
+ **Workstyle:** This is remote position.
+ **Workdays and Hours:** Monday – Friday; 8:00am – 5:00pm Eastern Standard Time (EST).
+ **Travel:** Up to 5 % to onsite team engagement meetings possibly outside your state of residence.

**Pay Range**
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$86,300 - $118,700 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
