Position Purpose
Quality Documentation Specialist ensures that the appropriate core measure data collection is completed timely, submitted while assuring monitoring systems are in place to promote quality care. This individual is responsible for the collection of required data to support the improvement activities surrounding the measures. Accurately abstracts the core measure data for required submission to meet regulatory requirements as well as the generation of standard reports for appropriate stakeholders. The Quality Documentation Specialist establishes effective working relationships with medical staff, nursing, and administration to support the quality of care and patient safety objectives of the organization.
What you will do
Collaborate with stakeholders in collecting and evaluating clinical data to achieve positive clinical outcomes and strategic goals
Collaborate with department/service line leadership to develop and implement data collection systems that have accessible and retrievable formats to use data to plan, deliver and evaluate patient care and services
Collect, validate, and enter data into Core Measure database according to Joint Commission/CMS requirements and methodologies
Review documentation in each DRG specific medical record for compliance with Core Measure abstraction criteria
Facilitate collection, review, and analysis of data for improvement opportunities
Minimum Qualifications
Graduate of an accredited nursing program; BSN preferred
Two years’ experience in quality improvement, data collection and analysis in a hospital setting required
Experience with Core Measures and/or data abstraction for proprietary databases strongly preferred
Current RN CT licensure
Proficiency in data collection and data analysis for quality improvement.
Familiarity with basic statistics, data analysis and statistical process control (SPC)
