We have an exciting opportunity for a Senior Health Outreach Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

Full Time Salary Range: £32,002.35 - £34,214.20
Allowance: Inner London Weighting £4133.14 (paid pro rata)
*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
ResponsibilitiesAbout the role:
Hold a caseload of clients with complex and multiple needs, including chronic physical health conditions, severe mental illness, and substance use issues. Conduct assertive outreach during unsociable hours to engage individuals sleeping rough, building trusting relationships over time.Develop and implement person-centred support plans, ensuring clients are linked into appropriate health, housing, and social care services. Work closely with the Health Provision in borough to coordinate holistic care plans and ensure continuity of care.Attend and contribute to multi-agency case conferences, safeguarding meetings, and health and housing panels. Support clients to attend medical appointments, understand treatment plans, and adhere to prescribed care. Advocate for clients within health systems, helping to reduce barriers to access and improve engagement with primary and secondary care. Provide health promotion and harm reduction advice, including around wound care, medication adherence, and overdose prevention.
About you:
Experience working with people experiencing homelessness or rough sleeping.Understanding of health inclusion and evidenced experience of supporting vulnerable adults to access health servicesStrong understanding of physical and mental health issues affecting this population.Proven ability to manage a complex caseload and work autonomously in the community.Excellent partnership working skills and experience of multi-agency collaboration.Knowledge of safeguarding procedures and trauma-informed approaches.
