Shift Worked: 8:00AM - 4:00PM [Monday - Friday] Weekly Hours: 37.5 FLSA Status: Non Exempt Pay range: $30.00 - $30.00 A. POSITION SUMMARY: Designs, implements and monitors individual program plans for persons supported. Encourages, implements and facilitates independence, inclusion, individuality and productivity. Ensures compliance with OPWDD regulations for all documentation of services. Performs internal audits. B. POSITION DUTIES: 1. Participates as a team member to develop and implement participant’s program plans for a limited caseload. 2. Coordinates and provides direction to team members in the assessment of program participant’s needs and development of appropriate individualized program plans that are in compliance with agency policy as well as State and Federal regulations. Assures integration of services with residential providers. 3. Ensure that day programs promote person-centered approach in service planning and delivery. 4. Develop, implement and monitors the program plan for each person receiving services. 5. Trains and supervises Habilitation Coordinators on goal/outcome implementation/paperwork 6. Participates in screening new individuals, follows up on admission and discharge (DDP1’s) as well as updates DDP2’s via Choices. 7. Schedules and attends all semi-annual and special conferences for participants. 8. Ensures that all Habilitation Coordinators duties and responsibilities are carried out. Ensures their caseloads are managed appropriately and are in compliance. 9. Performs internal audits of all documentation required for billing purposes. 10. Completes weekly/monthly activity schedules. 11. Participates in group activity routines when needed. 12. Research inclusion opportunities and works with Assistant Director to implement. 13. In the absence of the Directors, assumes role and responsibilities. 14. Promote self-advocacy for people that participate our programs. 15. Attend all mandated trainings 16. All other related duties as assigned. C. REQUIREMENTS: 1. Bachelor’s degree in Health or Human sciences or related field required. 2. Minimum 1 years’ experience working as a Habilitation Coordinator or equivalent position in a program for people with developmental disabilities. 3. 2 years of supervisory experience preferred. 4. Working knowledge of Microsoft Office Applications required. 5. Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
